Registering with the surgery

To register with the surgery:

We request that you provide proof of ID to complete the registration process via a photo ID presented to the surgery in person and an address ID dated within the last 3 months (to ensure that you are living within our practice boundary).

Please note that your registration will be processed once we have received the appropriate ID and completion of the new patient form below. If you are prescribed regular medication, please ensure that you book in with our clinical pharmacist once registered to enable this to be added to your notes for repeat ordering

Registration will not be completed until proof of identity has been provided.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Personal GP All of our patients have been allocated a named, personal GP. Patients wishing to find out their named GP can contact the practice for this information.

Patients will be advised to see their personal GP for most matters to ensure continuity of care. For urgent medical advice, patients may need to speak to another GP at the surgery.
